Abstract
Surveillance of the infectious risk in neonatal unit in specialized hospital mother-child
Tlemcen 2009-2010
Introduction
The aim of this study was to determine the incidence of nosocomial infections, epidemiological
profile, their place in neonatal morbidity and mortality and to identify the principal factors
associated in a neonatal unit polyvalent Tlemcen.
Materials and methods
The CDC criteria for less than 12 months are used for an incidence study relating to a cohort of
neonates admitted between 2009 and 2010, incorporating a case - control study to understand
the risk factors and diagnostic quality of the unit, based on the study of the microbial ecology
of the service, the Ishikawa diagram, and an audit of hand washing.
Results
A total of 2786 neonates admitted ,472 deaths and 97 infectious episodes are recorded, an
incidence density of 6.27 per 1000 days, a cumulative incidence of 33.7 per 1000, a sex ratio of
0.9. Sepsis is predominant site (76%). The risk factors found are prematurity (p = 0.002), low
birth weight (p <10

), a small height and head circumference (p <10

). The risk of nosocomial
infection associated with severe prematurity is higher (OR = 11.26, p <10

). The presence of
peripheral venous Catheter, oxygen treatment, artificial feeding and the cramming are risk
factors. The risk of death is higher in cases of nosocomial infection (OR = 7, p <10
-3
). The
duration of hospitalization was significantly prolonged in the cases (p <10
-3
).
Conclusion
The nosocomial infection is an important disease in neonatology, a major risk of mortality and
cause additional costs, related to the extension of stay. The prevention of these infections
requires a reorganization of the NICU according to national standard (national perenatal
program), the fight against prematurity and application of the rules of basic hygiene.
